Critical conditions are characterized by severe hypermetabolism , catabolic metabolism, immune dysfunction, and the risk of developing nutritional deficiency already on the first day of stay in the intensive care unit (ICU). Current recommendations emphasize the early initiation of enteral nutrition, preferably within 24–48 hours after stabilization of hemodynamic parameters, which helps maintain the intestinal barrier function, reduce bacterial translocation , and reduce the risk of infectious complications and multiple organ failure. Its pathogenesis includes a complex of hypoxic -ischemic, reperfusion , inflammatory and metabolic disorders leading to multiple organ dysfunction in patients after successful cardiopulmonary resuscitation. The main causes of circulatory arrest were acute coronary pathologies and cardiac arrhythmias. The most pronounced clinical manifestations included hypoxic encephalopathy, myocardial dysfunction, infectious complications and metabolic disorders. Complex therapy using targeted temperature therapy, hemodynamic stabilization, nutritional support, infection prevention, cardiological and neurological correction, as well as early rehabilitation has demonstrated a positive effect on treatment outcomes. The importance of a multidisciplinary approach to managing patients with PRS aimed not only at survival but also at restoring quality of life is emphasized. This article reviews the role of objective monitoring of the depth of anesthesia in reducing the incidence of postoperative complications, including cognitive dysfunction, delirium, cardiorespiratory complications, and increased mortality. Accurate titration of anesthetics according to neurophysiological parameters helps avoid hypnotic overdose and hypoanesthesia , thereby reducing the level of systemic stress, inflammatory reactions, and the risk of perioperative hypotension. And the use of monitoring the depth of anesthesia helps to reduce the time of awakening and accelerate postoperative rehabilitation, especially in elderly patients and those with comorbidities. This dysregulated hemostatic response not only precipitates microvascular thrombosis and subsequent tissue ischemia but also exhausts platelets and clotting factors, leading to paradoxical bleeding. Particular attention is paid to the influence of age and gender differences, as well as somatotypic factors on the size, volume and functional activity of the organs of the hepatobiliary complex. The article highlights the modern possibilities of ultrasound diagnostics as a leading non-invasive visualization method that allows assessing the structural and functional parameters of organs in dynamics, conducting early diagnostics of diseases and monitoring the effectiveness of therapy.
